Constipation is a common medical problem, the most common cause of which is inadequate dietary fiber intake, often coupled with insufficient exercise. Chronic constipation frequently causes abdominal pain. While the pain from constipation can occur anywhere in the abdomen, the most common location is the lower left quadrant. More serious causes of pain in the left abdomen may lead to constipation, but these causes will almost always be accompanied by additional symptoms.

Conditions Diverticula Disease Diverticula are small pouches that stick out from the large intestine. Although around half the population will have diverticula by the time they are 50, 3 in 4 will never experience any related symptoms. However, the other 1 in 4 may experience abdominal pain and diarrhoea, which is known as having diverticular disease. Diverticulitis can also occur, which is when the diverticula become inflamed or infected. Diverticular disease may be characterised by a change in normal bowel habits, such as constipation or diarrhoea, bloating, or bleeding from the rectum. Pain in the abdomen (stomach) will often be intermittent and felt on the lower left-hand side. Symptoms of diverticulitis also include abdominal pain, constipation and bleeding; but may also encompass nausea, vomiting, frequent and painful urinating, and a high temperature. Complications arising from diverticulitis have been known to include abscesses and fistulas. Both diverticular disease and diverticulitis can be caused by a lack of fibre in the diet, as this is what is thought to cause the development of diverticula. Fibre makes stools softer and easier to push out. Straining to push out harder stools can weaken the muscle: this allows the inner layer of the colon to push through, thus creating the diverticula. Risk of diverticular disease may be increased by smoking, being overweight, lack of physical activity, and having a history of constipation. Infections causing diverticulitis may occur when a hard stool gets stuck in one of the pouches, giving bacteria a chance to multiply and spread. As many conditions have similar symptoms to diverticular disease and diverticulitis, GPs may recommend blood tests as a way of ruling out these other conditions. A colonoscopy, whereby a tube with a camera on the end is inserted up your rectum and into the colon, can confirm the presence of diverticula. Alternatively, a barium enema X-ray can be undertaken, which involves squirting barium liquid up a tube into your rectum. Diverticular disease can generally be treated with painkillers such as paracetamol, although ibuprofen and aspirin are not recommended as they may upset your stomach. A high fibre diet may also help manage the symptoms, however in cases where blood loss is severe, a blood transfusion in hospital may be necessary. Diverticulitis may also be treated at home, with antibiotics to combat the infection and paracetamol to address the pain. In more serious cases, for example where your GP suspects complications or you are dehydrated, treatment may be administered in hospital. This will involve antibiotic injections and being hydrated and nourished through a drip. Surgery is rarely recommended, however if it were to be undertaken it would involve removing the affected area of the colon.

Less commonly, more serious conditions can cause left abdominal pain accompanied by constipation. These conditions include tumors, diverticulitis, volvulus and nerve damage. These conditions will almost always be accompanied by additional symptoms, such as unexplained weight loss, fever, numbness in the inner thighs or genitals, blood in the stool or extremely intense abdominal pain. Patients experiencing these symptoms in combination with constipation and abdominal pain should seek prompt medical attention.

Medications are also a common cause of constipation. Opiates, such as Vicodin, Percocet and Oxycontin, certain iron supplements, some antipsychotic medications, antihistamines, and some heart and blood pressure medications, such as Norvasc and Procardia, can cause chronic constipation. A patient who is taking one of these medications and is having problems with constipation should talk to a doctor about it, since it may be possible to substitute a drug that is less likely to cause this problem.

I am 26 and I have been dealing with a dull ache in my left side for about 6 months now. It started as a sharp ache, in which I went to the gyno and got an ultrasound – all clear. He mentioned it could just be constipation, which I believed because I have always had issues with constipation. However, the dull ache is pretty constant now, and my mind starts to assume the worst. About 2 months ago I got blood work and urinalysis done – all normal. My GP referred me to a GI who told me I had IBS-C without doing any tests. She had me do the prep for a colonoscopy (yuck!) just to clear out my system, then start taking miralax everyday to have a BM daily. The miralax has helped with BMs, but makes me bloated and gassy . The worst part for me is the constant dull ache on my left side – usually right below my ribs. The pain sometime makes my back hurt right below my ribs as well. I am set to have a sigmoidoscopy in a few days – not looking forward to it, but hoping for some answers.

You are not alone! I’m 66 and I’ve been dealing with stinging pain in my left side for over a year now. It started as an ache about the size of a postage stamp. Now, it’s the entire left side of my abdomen. I can hardly stand more pain. It’s so bad that I can’t even touch my skin when the pain is peaking. I’ve had two colonoscopies in three years, three sonograms and two CT scans and a Nerve Study and an MRI done. I have 3 or 4 bouts of pain every day and one at night. I’m in pain more hours than not. I have spasms, too. I haven’t been able to find anyone with symptoms exactly like mine. My life revolves around this pain. I’m looking for answers, but wondering how much more pain I can take. It’s almost like I’m having labor pains and being electrocuted at the same time. It’s debilitating and makes me feel exhausted. I’m doing acupuncture twice a week.
